To schedule a new patient appointment, please contact our office directly at 970-252-7444 or send us a message through our quick and easy contact form below.
After scheduling an appointment, you will receive a registration link from eDerm. Login with the patient’s last name and date of birth. If you didn’t receive the email, please check your Spam Folder. If you still can’t find the email or can’t login, please call the office.
New Patient Registration
Welcome! We’re excited to care for you. All new patients are required to complete our New Patient Registration forms. These forms allow us to gather important medical and contact information to ensure a smooth check-in and the best possible care.
New Patient Forms are available on our website homepage under Patient Forms. We will also send you a Self-Registration link that you can complete prior to your appointment. This will help ensure no delays at check-in.
If you are a new patient, we ask that you arrive 20 minutes ahead of your appointment.
